About Health & Wellbeing


The Health Team work with over 400 organisations to improve a person’s health, well-being and employability. We also work directly with clients via our Social Prescribing Link Workers, Wellbeing Coaches, Welfare Coaches, Employment Coaches, Development, Volunteering and Centres.

We work 1-2-1 with clients and also deliver lots of group activities that are co-produced with SOAR’s volunteers, including:

  • Chronic Pain groups

  • Women’s Only Support group

  • Swim Bus

  • Craft groups

  • Cook & Eat sessions

  • Springboard Social Cafes

  • Dementia Social Groups

  • Carer Support Groups

  • Mental Wellbeing Social Groups

Social Prescribing

SOAR’s Social Prescribing Service works across 22 GP Surgeries (part of North Locality) in North Sheffield. 

Key Aspects:

  • Social Model of Health

  • Range of interlinked services ‘no wrong door policy’

  • Co-location of workers & services (across 3 sites)

  • Experienced local workers (78% North Sheffield residents)

  • Delivered in non-clinical, ‘community-based’ settings

How the service works

Step One

A referral is made to SOAR by either your GP Practice, a professional service, or self-referral. 

Step Two


A member of the Social Prescribing Team will triage referrals and carry out assessments over the phone or face-to-face, to find out how we can help and then use that information to signpost to a service who can best meet those needs. This service might be internal to SOAR, or external to a citywide partner.

Step Three


If referred internally to SOAR, SOAR Wellbeing Coaches and Welfare Coaches spend time with those referred, to understand, build confidence, plan, support, and/or refer them on to additional support. The skills of these staff are key to the success of this work.
