Avanda Miller - Social Prescribing Link Worker
Avanda joins the team as a Social Prescribing Link Worker for the Foundry Primary Care Network. Our Link Workers triage referrals and carry out assessments over the phone or face-to-face, to find out how we can help and then use that information to signpost to a service who can best meet the individual's needs.
"I will be understanding the needs of the people I'll be supporting, and helping them come up with a personalised plan to make sure they are referred to the right organisation, social groups, etc.
I am hoping to learn more about local organisations and reconnecting to people in the community.
Outside of work, I enjoy cooking, reading, listening to music. When the weather is good being outdoors! I also love doing anything creative like water colouring and drawing with my nephews."
